OFFICE CLOSING

Effective November 22nd, 2011, our office located at 1021 Central Ave in Middletown will be permanently closed.

Please make sure the agency has your correct phone number.

If you are a spend down customer please mail your spend down payments to the address listed below (check or money order only). Please allow ample time for the mail to be received by our office, as this could delay the date of release.

There are 4 ways to apply:
  1. BEST: Online at ODJFSBenefits.ohio.gov (also for reporting changes).
  2. BETTER: Download the 7200 application from our site here.
  3. BETTER: Call and request an application packet be mailed to you.
  4. GOOD: In person in our office.
You can provide requested verifications by mail at 315 High St, 8th Floor, Hamilton, OH 45011, by fax at 513-887-4334 or 513-425-8777, or in our office.
