The Butler County Health Department maintains birth and death certificates for Butler County, Ohio from 1909 to the present, except for births or deaths which occur in the cities of Hamilton and Middletown. 

You may purchase a certificate in person or via mail.  No personal checks will be accepted.  To download the birth and death certificate applications please visit our Forms page on this website.

Effective October 16, 2009, there is a statutory fee of $25 per certified copy of a birth or death certificate.
