The Butler County Health Department maintains death certificates for Butler County, Ohio from 1909 to the present, except for deaths which occur in the cities of Hamilton and Middletown.

For deaths in the cities of Hamilton or Middletown, the city health departments are the appropriate agencies to contact to obtain a death certificate.
